Intravitreal injection of Anti-VEGF (Avastin) has shown beneficial in cases of refractive macular edema (diabetes, CRVO), SRNVM (wet ARMD), etc. Refractory macular edema, or swelling around the macula, is edema that affects vision but does not respond adequately to the usual treatment methods. It can occur with conditions such as central retinal vein occlusion and diabetic retinopathy. Without effective treatment, vision loss could progress and become permanent. Age-related macular degeneration (AMD) is the leading cause of blindness in people over 50 years of age. It is caused by the breakdown of the central portion of the retina (the nerve layer part of your eye that works like the film in a camera to pick up the picture) called the macula. The macula is responsible for the fine central vision in the eye that is needed for driving a car, reading fine print, recognizing faces, etc. There are two types of macular degeneration: dry and wet. In the wet form of AMD, abnormal blood vessels grow in the back of the eye. Sometimes these vessels leak blood or fluid that causes blurred or distorted vision. ![]() | Dr. Chan Discusses the VEGF-Trap Agent Aflibercept Emily Chan, MD, PhD, Assistant Professor of Medicine, Vanderbilt University Department of Medicine, Division of Hematology/Oncology, Vanderbilt-Ingram Cancer Center, discusses the investigatory agent aflibercept, a vascular endothelial growth factor (VEGF)-trap agent. Aflibercept acts as a decoy receptor, which causes pro-angiogenic VEGFs to bind to its ligands rather than the correct VEGF receptors. Trapping VEGF with false receptors prevents tumor angiogenesis. Historically VEGF has proven to be an active target in colorectal cancer and is successfully inhibited at different points by other agents, such as bevacizumab. Although aflibercept is not currently an approved agent promising data was released supporting its use in metastatic colorectal cancer at the 2011 ECCO/ESMO meeting.
